FOSTER CARE PROVIDER AGREEMENT:

Foster Care Provider Agrees:*
- I agree to provide care for the pets until the owner's return. This includes adequate food, water, playtime, companionship, shelter, veterinary care, keeping the pet safe, and lots of love.
- The pets will live in my home as a first-hand family members, and not be kept outdoors.
- I agree to remain in contact with the owner, and periodically send photos and updates on the pets' well-being and adjustment to my home.
- I agree to closely monitor the pet's health and well-being, and notify the owner if any issues arise which may need veterinary care.
- I agree to immediately notify the owner if my address or other contact information should change.
- I agree that small children (aged 0 to 6) will be supervised when in the presence of the pet.
- I agree to surrender the pets to the owner upon their return.
PET OWNER AGREEMENT:

Pet Owner Agrees:*
- I agree to pay the sum of $150 each month in consideration for their providing good care to my pets.
- I agree to provide and/or reimburse the foster care provider for all supplies needed in the day to day care of my pets. This includes food, cat litter, treats and any other supplies mutually agreed as necessary.
- I agree to pay for any veterinary care the pets may need as mutually agreed as necessary.
- I agree to keep in touch with the foster care provider, and let the provider know if I will be unreachable for any period of time.
- I agree to keep the foster care provider informed as to my expected date of return and retrieval of my pets.
By signing below, I agree to all conditions set forth in this foster care agreement.
